1.GENERAL STATEMENTS

This form hereby has been prepared in accordance with Article 11 and 13 of Personal Data Protection Law (“PDPL”) numbered 6698.

The persons with personal data (“Applicant”) who was defined as the relevant person in PDPL were entitled with the rights specified in Article 11 of PDPL; the applications to be made in relation to these rights must be conveyed in written to İdamar Mermer Granit Ticaret Limited Şirketi (“Company”) which is responsible for data pursuant to subparagraph of Article 13/1 of the same law. In this context, the application to be made to our company in written can be done by printing out this form by way of using application methods stated below.

The expression “Information Request within Scope of Personal Data Protection Law” must be written in subject of e-mail. The channel which are specified above are “written” application channel according to 1st subparagraph of Article 13 of PDPL. In case that Council of Personal Data Protection determines other methods in the further process, how to receive applications through these methods will be announced by our company.

Your request will be responded by our company according to the feature of your request “as soon as possible and within thirty days at the latest” in accordance with 2nd subparagraph of Article 13 of PDPL. Our responds shall be sent you in written or electronic media pursuant to the relevant Article 13 of PDPL.

The information and documents requested from you must be conveyed to us completely and correctly. In case that the requested information and documents are not provided as necessary, there may be problems in performance of the researches to be carried out in full and qualitatively based on your request. In this case, the legal rights of our company are reserved. For this reason, the relevant form including complete and requested information and documents must be sent according to the feature of your request.

5.DECLARATION OF APPLICANT (Article 13/1-3 of PDPL)

In line with the request I mentioned above, I request you to notify me through evaluating my application I made to your company in accordance with Article 13 of PDPL.

• I would like it to be sent to my e-mail address. (In case that you prefer e-mail method, we will be able to reply to you quicker.)
• I would like to receive by hand. (In case of receipt by proxy, a notarized letter of attorney or certificate of authority including private authority is required.)

This application form is hereby issued by determining your relation with our company, identifying your personal data processed by our company entirely, if available in order to respond to your relevant application correctly within legal period. With the objective to eliminate legal risks arising from illegal and unfair data sharing and to ensure especially security of your personal data, our company reserves the right to request additional documents and information (Copy of identity card or driver’s license etc.) for identification and authorization.
In case that the information related to your requests you conveyed within the scope of the form is incorrect and outdated or an unauthorized application is made, our company declines responsibility due to requests in connection with the aforementioned incorrect information or unauthorized application.
